PRP services are rehabilitative, meaning they focus on helping individuals restore, strengthen and maintain functional skills rather than treating symptoms through therapy alone.
The goal of PRP is to help individuals:
- Increase independence
- Improve daily functioning
- Strengthen coping skills
- Reduce psychiatric hospitalizations
- Improve community integration
- Build social and life skills
- Support recovery and wellness
Our type of care is achieved through collaboration with the consumer, family or other support (guardian, parents, etc.), and their mental health provider. By formulating this type of team, PRP helps people live as independently and successfully as possible despite mental health challenges.

Individualized Rehabilitation Plans (IRPs)
This includes both direct and indirect rehabilitative support services to include structured
activities, skill-building sessions, and therapeutic interventions designed to enhance clients’
functional independence, emotional well-being, and community integration.
Role Model and Life Skills Coaching
Consistent coaching in life skills (e.g., personal hygiene, time management, financial literacy),
social skills (e.g., communication, conflict resolution), and coping strategies (e.g., emotional
regulation, stress management) to foster client self-sufficiency and resilience.
Individual and group sessions
Safe, structured and trauma-informed environments that promote emotional safety, respect, and inclusivity whether confidential to tackle personal challenges or collaborative to gain new perspectives and connections.
Preventive Mental Wellness Classes and Workshops
Our goal is to assist individuals in regaining effective impulse control, cultivate self-esteem, improve communication, teach the skills necessary for daily independent living (inclusive of appropriate nutrition, personal grooming, money management, occupational preparedness), as well as prevention of illness and recurrence of symptoms related to their mental health. In addition, we endeavor to utilize strategies that reduce hospital admissions.
Treatment Referrals
Our treatment referrals are to trusted providers and organizations within the community. This may include psychiatric services, medical providers, substance use treatment, specialized therapies, or other supports.
Case Management Services
Case management services help individuals navigate complex systems of care. We assist with coordinating services to help individuals remain engaged in treatment and community life.
Connection to Community Supports and Resources
We actively connect individuals and families to community-based support that enhances well-being and stability. This can look like facilitating a mentor-mentee connection with someone nearby, hosting fun events for people to meet others or educating clients on resources in their community to take advantage of. By strengthening connections and educating others, we support holistic care that extends beyond the clinical setting.
